Please complete this form to submit a short student bio (3 to 4 sentences is sufficient).  You can include hobbies, something interesting about the student, achievements, favorite music/film/book/food, how the student chose the instrument, or other fun and interesting facts.  Also, please upload a photo (head shot) of the student).

Example:  Ethan Rivera, age 12, is a 6th-grade cellist at Lincoln Middle School. He has been studying cello for two years with Mr. Cello Teacher. Ethan loves performing with his school orchestra and recently participated in his first solo competition. Outside of music, he enjoys building model airplanes and hiking with his family.
